This form is used by someone who has been told they are a vexatious litigant and needs permission from the court leader to file a new case or document. It asks the court for approval to file a specific document and requires the person to explain why the new filing has merit and why it is not intended to cause trouble or delay.
The court describes it as: If you are subject to a prefiling vexatious litigant order (an order that you cannot file a new case in a California court without first getting permission of the leader of the court), ask the court for permission to file a new document in a California court.
