Proof of Service by Mail (Small Claims)

This form is used to tell the court that certain small claims documents were sent out through the mail. It asks the person mailing the documents to fill out details about themselves, what was mailed, and when and where the mailing happened.
The court describes it as: Tell the court that you served Small Claims forms by mail.
