This form is used to ask a judge to make a decision in a small claims case, either before or after the trial. It also allows a person to respond to someone else who has already asked the court for a decision.

The court describes it as: Ask a judge to make a decision about something before or after your Small Claims trial date. You can also use it to tell the court your side when someone else in the case has filled out this form to ask the judge to make a decision.

Who fills out this form?
The person asking the court for an order fills out page 1. If you receive the form, you fill out page 2 to answer it.
Do I have to serve this form?
If you ask for the order before your trial, you must mail or deliver a copy to all other people in the case. If you ask after the judge has decided, the clerk will mail it.
