Proof of Mailing (Substituted Service) (Small Claims)

This form is used to tell the court that you mailed the required Small Claims documents using a method called substituted service. It asks for details about who received the papers and when they were mailed.

The court describes it as: Tell the court who mailed your Small Claims forms if you used substituted service and had a different server mail the papers. If the same server handed and mailed the forms, you do not need to use this form.
