Notice of Remote Appearance

This form is used to tell the court that you or a witness plan to appear at a court hearing by phone or video, instead of coming to the courthouse in person. It also helps you show that you told all the other people involved in the case about your plan to appear remotely.
The court describes it as: Tells the court that you or a witness intends to appear at a court proceeding remotely, either by telephone or video, rather than in person. May also be used to tell other parties that you will be attending remotely, and to tell the court how you told each of the other parties about the remote appearance. Explains, at page 3, how and when to use the form, including deadlines.
