This form is a confidential list used to tell the court what personal numbers, like social security or bank account numbers, were taken out of documents being filed. It helps the court know what information was hidden for privacy reasons.
The court describes it as: If a court orders it, give the court a confidential list of the social security and financial account numbers that were required to be left out or taken out of papers you filed in the court.
