This form is a Notice and Request for Ruling used when someone has already filed a petition for a writ of habeas corpus. It asks the court to make a decision on the original petition if more than 60 days have passed since it was filed.
The court describes it as: If more than 60 days has passed since you filed a petition for a writ of habeas, ask the court to make a decision on your petition.
