Declaration of Disclosure

This form is a Declaration of Disclosure used in family law cases to list all the financial papers and information that one party gives to the other party. It is a statement of facts that must be exchanged between the parties.

The court describes it as: List all the financial documents and information on this cover page that you give (disclose) to your spouse or domestic partner as part of the first or final set of disclosure documents.
