This form is used to ask the other side to officially agree (admit) that certain facts are true or that certain documents are real. If something is admitted, it might not need to be proven later in court.

The court describes it as: Tell the other side to admit that a list of facts are true or documents are genuine. If admitted, a fact will not need to be proven at trial, or a document will not need to be authenticated.
