Stipulation Converting Legal Separation to Divorce

This form is used when two people who have already been legally separated want to ask the court to change that legal separation into a divorce. It is a voluntary agreement between the parties.
The court describes it as: After a legal separation has been granted, the parties may use this form to have the legal separation converted into a judgment of divorce. Voluntary form.
