This form is used to tell the court if you need a written record (transcript) of what was said during a previous court hearing for an appeal. It asks you to check boxes to show what kind of transcript is needed or if one is not needed at all.

- What is a transcript?
- A transcript is a word-for-word written record of everything that was said in court. If you did not appear in court, a transcript is not necessary.
- What happens if I check the box for a transcript but don't fill out the bottom part?
- The statement will not be accepted if you check the box for a transcript but do not fill out the bottom section and get the court reporter's signature.
