You must fill in your name as the 'Petitioner' and the opposing party's name as the 'Respondent.' You also need to provide details about the Canadian domestic violence protection order, such as the case number and the issuing Canadian court.
Do I need to sign it?
The judge will fill in the date and time the order was granted and sign it. The 'Petitioner' must sign the bottom of the form with today's date, and the 'Respondent' will also sign if they attend the hearing.
