Proof of Service by Mail

This form is used to prove that certain documents were sent to another person by mail. It requires the person filing it to state that they are not a party to the case and to check boxes showing how service by mail was allowed.

- The party who received these documents must file the original Proof of Service by Mail with the court clerk.
