- The sender must include copies of the Summons and Complaint or Petition, payment for the sheriff's fee, and a self-addressed, stamped envelope for the Proof of Service.
- Does this form cost money?
- The sender must include payment for the sheriff's fee, unless they have an Order for Waiver of Court Fees.
