Process Service Memorandum

This form is used to give instructions to a sheriff or private process server about how and when to deliver court papers to another person in a case. It allows the person filing the form to provide specific locations, like work or home, for the service to happen.

- The form is for the party filing the documents, which is listed as the Petitioner on the form. If a nonlawyer helps fill it out, they must also complete sections at the bottom.
- What does the filer need to do after filling this out?
- The original form must be filed with the clerk of the circuit court where the petition was first filed. A copy must also be attached to the main service form (Form 12.910(a)).
- Can I use this form if I need to keep my address private?
- If the filer fears that giving out their address is unsafe, they should use a different form, Form 12.980(h), and write 'confidential' on the petition.
