Waiver of Hearing, Waiver of Final Conservator's Report, Waiver of Audit and Approval of Schedule of Distribution

This form allows a person to agree to several things related to a conservatorship, including giving up the right to see the final report from the conservator and approving how the estate's assets will be given out. It is a formal agreement signed under penalty of perjury.
