Is this form required?
The JDF 601 case cover sheet is required for all District Civil (CV) actions filed on or after July 1, 2004, and must be filed with the initial complaint.
What happens if I don't file this form?
If you fail to file a Case Cover Sheet with your initial papers, the Court may notify you and could impose sanctions, such as striking your filing.
