Notice and Acknowledgment of Receipt

This form is used to tell the court that the other people involved in a gestational carrier agreement received the legal papers to start the case by mail. It requires signatures from both the person sending the papers and the person receiving them.

The court describes it as: Tell the court that you and the other party or parties in the case agreed to send (and receive) the legal papers to start your case by mail.

The sender fills out items 1 through 5 and signs it before mailing. The recipient fills out item 6, and possibly item 7, and then returns it to the sender.

If the documents include a summons and you do not return this form within 20 days of the mailing date, you might be responsible for the costs of serving you later.
If I disagree with the request, what should I do?
If you do not agree with what is being requested, you must submit a completed Response using form SUR-120 to the court within 30 calendar days.
