Release and Receipt of Superior Court Records

This form is used when a court gives away court records it planned to destroy. One part of the form lists the records being given out, and the other part is for the receiving group to sign and say they got those records.
The court describes it as: A court that gives court records that it was going to destroy to an entity uses this to list the records that were given to that entity and the entity uses this to acknowledge that they received those records.
