This form is used to tell the court and other people that you do not want a court hearing or trial to happen by phone or video. It is also used if you want another person or witness to appear in person instead of remotely.
The court describes it as: Tells the court and other parties that you want to appear at a court proceeding in person, rather than by telephone or video, or that you want another party or witness to appear in person and why. Explains, at page 2, how and when to use the form, including deadlines.
