Cause of Action—Fraud

This form is used to tell the court that you believe the other side made false statements or intentionally left out important facts that you reasonably trusted, and because of this, you were harmed. It must be attached to a main Complaint form to be used.

The court describes it as: Tells the court and the other side that you contend that the other side made a false representation or intentional omission that you reasonably relied upon, and were harmed as a result (fraud). Must be attached to a Complaint form to use.
