- Who must fill out this form?
- The person completing this form must be 18 years of age or older and cannot be listed as either the party asking for protection or the party to be restrained. The person must personally give copies of the documents to the other party. Great care must be taken to fill in the required information, including the date and time of service.
- What does this form prove?
- This form proves that the person signing it personally gave copies of a list of specific documents to the named party. The person must sign under penalty of perjury that the information is true and correct.
