Instructions for Requesting a Hearing to Review Waiver of Presumptive Transfer of Specialty Mental Health Services

This form provides instructions for asking the court to hold a hearing to review if the county where a child lived before being placed in foster care should keep responsibility for the child's mental health services, instead of the county where the child is now living.

The court describes it as: Explains the process, for a child who has been placed in foster care out of the county where they lived before being put in foster care, to ask that their mental health services are provided by their home county and not the county of their foster home.

If a hearing is granted, the transfer of services is paused until the court makes a decision. Services will continue to be provided by the child's or nonminor's home county until the court rules.<0xC2><0xA0>
