Confidential Conservatorship Care Plan—Part 1

What does this form require when filing?
When the care plan is finished, the conservator must sign page 4 of this form and page 2 of the related medical form (GC-356), and then file both parts with the court.
What happens if I miss the deadline?
If a completed care plan is not filed by the required deadline, the court could remove the conservator, order a penalty, or refer the person to an investigation.
