- Everyone asking the court to extend the guardianship must sign this form. The ward and the proposed guardian must also sign it. The ward must be at least 18 but not yet 21 years old and not a petitioner to sign it.
- What does this form ask the court to do?
- It asks the court to extend the appointment of the guardian of the person past the ward's 18th birthday and potentially issue new Letters of Guardianship.
