Declaration Regarding Address Verification—Postjudgment Request to Modify a Child Custody, Visitation, or Child Support Order

This form is a sworn statement used to tell the court that the person filling it knows the current address of the other parent. It is used when asking the court to change an existing order about child custody, visitation, or child support.
