Request to Enter Default (Family Law—Uniform Parentage)

This form asks the court to officially record that the other person in the case has failed to respond to the initial filing. It is used to ask the judge for a final decision based on either a signed agreement or what was originally asked for in the petition.

The court describes it as: Ask the court for a decision (order) that time has ended for your spouse or domestic partner to file a response to your petition. This allows you to ask the court for a judgment based on a signed settlement agreement or, if no agreement, based on what you asked for in the petition.
