This form is a notice and acknowledgment used to tell the court that one person has agreed to receive legal papers by mail. The recipient must sign the form to confirm they got the documents.
The court describes it as: Tell the court that your spouse or domestic partner agreed to (and received) by mail the legal papers to start your case.
