Proof of Service - Criminal Record Clearing

This form is used to tell the court that you have delivered certain papers to the prosecuting agency and other people involved in a case to help clear a criminal record. It asks the person filling it out to provide details about who received the papers and how they were delivered.
The court describes it as: Tell the court you had the prosecuting agency and other parties served with the papers you filed to clean your criminal record.
