Proof of Service (Appellate Division)

This form is used to tell the court that certain legal papers were delivered to the other side in an appeal case. It asks the person filing the form to list what papers were served, when they were served, and how they were served (by mail or in person).

The court describes it as: Tells the court that you had legal papers in a case in the Superior Court Appellate Division delivered to (served on) the other party either by mail or in person. Tells what papers were served and when they were served. For in person service, also says where the papers were served and who served them.
