Appellant's Proposed Settled Statement (Unlimited Civil Case)

This form is used by an appellant to provide a written summary of what happened during the spoken parts of a trial court case. It asks the appellant to summarize the testimony and evidence to the trial court and other parties involved in the appeal.
The court describes it as: Give the trial court and the other parties in an appeal an unlimited civil case your summary of the oral (spoken) proceedings in the case in the trial court (proposed settled statement). Use this form only if you are the appellant and you either chose to use a settled statement in your notice designating the record on appeal or the court made a decision (order) allowing you to use a settled statement.
