Respondent's Notice Designating Record on Appeal—Unlimited Civil Case

This form is used by a party who is responding to an appeal to tell the trial court what documents should be included in the official record for the Court of Appeal. It also lets the party ask for copies of the official records.

The court describes it as: Tell the trial court in an unlimited civil case, when another party is appealing, that you want more to be included in the official record for the Court of Appeal of what happened in the case. Use this form only if you are a respondent in the appeal and the appellant chose to use a clerk’s transcript or a reporter’s transcript for the official record. Also tell the trial court if you want a copy of the clerk’s transcript or a reporter’s transcript.
